hours notice to reschedule.
Payment for all missed sessions is required.
*************************************************************** Insurance Information In order to protect your privacy and ensure continued quality of care, Dr. Bouchard does not participate with managed care/insurance programs. At your request a receipt and diagnosis of your condition will be provided to you for a small administrative fee that you may present to your insurance company to attempt to obtain reimbursement.
