How Payment Works for Private Telephone Consultations
Costs:
Costs for office visits or distance telephone sessions are
$155 per 50 minute session hour for both individuals and couples.
Scheduling:
Sessions generally are scheduled once weekly.
Multiple sessions per week are also available for expedited results.
Sessions are typically scheduled from 48 hours to 10 days in advance.
Same day "spot sessions" or “crisis counseling sessions"
and after-hour/weekend sessions
are sometimes available for an additional $155 fee.
All fees for telephone and online services are due in advance
and are payable by credit card or check
through the secure PayPal webpage located on this site.
ALL RETAINERS ARE NON-REFUNDABLE
AND EXPIRE 90 DAYS FROM PURCHASE.

CLIENT AGREEMENT
To initiate and continue treatment
that best ensures therapeutic effectiveness
while under Dr. Bouchard’s care,
ALL clients must agree to be bound and adhere to the following
Terms & Conditions:
1. Client commits to meet for
uninterrupted consecutively scheduled weekly sessions
throughout the course of therapy until termination
(Unless prior arrangements have been agreed upon).
2. Client agrees to make full payment for all cancelled
or missed scheduled appointments.

Insurance Information
In order to protect your privacy and ensure continued quality of care, Dr. Bouchard does not directly participate with managed care/insurance programs. All fees for sessions must be paid to Dr. Bouchard directly by clients PRIOR to delivery of service.
At the client’s request, a billing receipt and diagnosis of your condition will be provided to you (for a small administrative fee) that you may present directly to your insurance company to attempt to obtain reimbursement.
Dr. Bouchard does not guarantee, control, nor in any way whatsoever accept liability for approval and payment of third party reimbursement to clients.
